2nd largest nursing home in Modoc County as of Q4 2023

In the fourth quarter of 2023, Surprise Valley Community Hospital D/P Skilled Nursing Facility stood out as the second largest nursing home in Modoc County, according to the Centers for Medicare and Medicaid Services (CMS).

Data shows that the nursing center had a total of 22 beds available, 14 less than the average in Modoc County as of the fourth quarter of 2023. They aided an average of 20.3 residents during that period.

Surprise Valley Community Hospital D/P Skilled Nursing Facility had an overall rating of 1 from the CMS, which lags behind the average of 3.2 recorded for nursing homes in California.

During the quarter, Modoc Medical Center D/P Skilled Nursing Facility stood out as the largest nursing home in Modoc County, with 50 beds available.

As of the fourth quarter of 2023, Surprise Valley Community Hospital D/P Skilled Nursing Facility has already spent a total of $31,498 in eight fines and suffered eight penalties.
